Dogs | Zoonoses
Which diseases a dog can pass to people, how they are transmitted, what people develop, what the dog itself shows, and how to break the chain: rabies, dermatophytosis, larva migrans caused by canine roundworm and hookworm, the two-way risk of *Salmonella* and raw meat diets, *Giardia*, leptospirosis, *Sarcoptes* mange, flea-borne tapeworm, echinococcosis, tick-borne diseases shared by dogs and people, *Brucella canis*, and the bacteria and resistant organisms associated with bites. Pregnant women, infants and young children, older people and immunosuppressed people are the high-risk groups. The last section sets out the household rules that matter more than memorizing disease names.
Dogs | Zoonoses
This content is compiled from the dog chapter of the veterinary animal encyclopedia. The same sign is handled differently in different species, and following advice written for another species can be harmful — confirm which animal you are dealing with first.
Basis in the teaching material (a single line): line 203 `• 公共卫生:食品安全、狂犬病等人兽共通疾病、抗菌药物管理。`
High-risk groups: pregnant women, infants and young children, older people, and immunosuppressed people (on chemotherapy, organ transplant recipients, HIV, long-term steroids or immunosuppressants).
| Pathogen (with English name) | Main route of transmission | Common presentation in people | Presentation in dogs | Key preventive points |
|---|---|---|---|---|
| Rabies (Lyssavirus) | Bites, scratches, saliva contact with mucous membranes and wounds | Extremely high case fatality once disease appears: WOAH states "once rabies symptoms appear the disease is invariably fatal both in animals and humans", (that is, case fatality after onset is close to 100%, and only intervention before onset is meaningful); post-exposure prophylaxis exists for the period before onset | Behavior change, the aggressive or the paralytic form, drooling, difficulty swallowing, neurological signs | Complete vaccination as required by the local competent authority; after a bite, wash the wound immediately with soap and water and seek medical care (for the person), and report as required locally; avoid contact with wildlife and free-roaming animals |
| Dermatophytosis (*Microsporum canis* and others) | Direct contact, environmental spores (hairs) | Ring-shaped red rash, scaling, itching | Focal hair loss, scaling, ring-shaped lesions (mostly in puppies and immunosuppressed dogs) | Treat early, clean the environment (spores persist), wash hands after contact, take care with children in the household |
| **Canine roundworm *Toxocara canis*** | Eggs in feces mature in the soil and are then swallowed | Visceral larva migrans and ocular larva migrans (which can cause loss of vision in children) | Abdominal distension in puppies, poor growth, worms brought up in vomit | Deworming schedules for puppies and dams, pick up feces immediately (eggs survive a long time in soil), wash hands after playing in sand or gardening, keep sandpits covered |
| Hookworm *Ancylostoma* spp. | Larvae penetrate the skin | Cutaneous larva migrans (a winding, red, swollen, intensely itchy linear lesion) | Anemia, black stool, can be fatal in puppies | Do not walk barefoot on damp soil or beaches contaminated with dog feces, keep the environment clean, deworm |
| *Salmonella* / *Campylobacter* / pathogenic *E. coli* | Dog feces, the dog's mouth and nose, raw meat diets and the surfaces they touch | Diarrhea, fever, abdominal pain (which can be severe in young children and immunosuppressed people) | May carry the organism without signs, or have diarrhea | Hygiene requirements are highest for those feeding raw: separate cutting boards and containers, wash hands after feeding, clean the bowl immediately; raw feeding is strongly discouraged when an immunosuppressed family member is in the household |
| *Giardia* / *Cryptosporidium* | Fecal-oral, contaminated water | Watery diarrhea, abdominal pain | Diarrhea (common in puppies and group housing) | Zoonotic potential varies by genotype; drinking-water hygiene, environmental cleaning, extra handwashing during treatment |
| **Leptospirosis, *Leptospira* spp.** | Urine of infected animals, contaminated water and soil, with rodents an important source | Fever, muscle pain, jaundice, hepatic and renal failure (Weil's disease) | Fever, vomiting, jaundice, acute kidney injury, pulmonary hemorrhage | Non-core vaccine (by regional risk), avoid standing water and rodent-infested environments, wear gloves and disinfect when handling the urine of an affected dog |
| *Sarcoptes scabiei* var. *canis* | Direct contact | Temporary intensely itchy rash (the mite usually does not complete its life cycle on people) | Intense itching, crusting on the ear margins and elbows, hair loss | Treat the dog plus every in-contact dog, clean the environment |
| Fleas and *Dipylidium caninum* | A child swallowing a flea carrying the tapeworm | Intestinal tapeworm infection | Perianal discomfort, rice-grain segments in the feces | Controlling fleas controls this tapeworm |
| *Echinococcus granulosus / multilocularis* | Swallowing eggs (from the dog's coat, from the environment); dogs are infected by eating raw offal or prey | Hepatic and pulmonary cyst disease in people, severe and difficult to treat | Dogs usually show no signs (which is what makes it dangerous) | In pastoral and endemic areas: do not feed raw offal or raw game, deworm regularly as locally advised, wash hands after contact with the dog |
| Shared vector-borne agents (*Ehrlichia*, *Anaplasma*, *Rickettsia*, *Borrelia*, *Babesia*, *Leishmania*) | A shared vector (ticks, sandflies); mostly not transmitted from the dog to a person directly | Fever, joint pain, rash and so on depending on the agent | Fever, low platelets, joint pain, anemia, kidney disease | Tick control is the upstream measure; check the whole body after outdoor activity (including ears, between the toes and armpits) and remove ticks correctly; tell the physician and the veterinarian about travel history |
| *Brucella canis* | Aborted material, reproductive tract secretions, semen, urine | Fever, joint pain, chronic infection (more severe in immunosuppressed people) | Abortion, infertility, orchitis, discospondylitis | Screen breeding dogs, wear gloves when handling aborted material, confirm that the breeder tests before buying a dog |
| *Pasteurella multocida* / *Capnocytophaga canimorsus* / *Bartonella* | Bites, scratches, licking of wounds | Wound infection that deteriorates quickly; *Capnocytophaga* can cause fatal sepsis in immunosuppressed and asplenic people | Usually normal oral flora | Do not let the dog lick wounds or faces; every bite needs medical assessment (for the person), especially bites to the hand and in immunosuppressed or asplenic people |
| Canine heartworm *Dirofilaria immitis* | Mosquitoes; people are an occasional dead-end host | Rare pulmonary nodules (usually an incidental imaging finding) | See §7 and §6.2 | Mosquito control and canine heartworm prevention |
| Resistant organisms (MRSP / MRSA and others) | Two-way transmission between people and dogs (within the household) | Skin and wound infections that are hard to treat | Skin, wound and ear infections that are hard to treat | Antimicrobial stewardship (original teaching material line 203): do not use leftover medication on your own, choose the drug on culture results, complete the course, and wash hands after touching an infected site |
Household rules (more important than memorizing disease names)
| Action | Notes |
|---|---|
| Wash hands | After touching the dog, after picking up feces, after feeding, after handling the bowl, and above all before eating |
| Pick up feces immediately | The key point at which roundworm and hookworm transmission is broken; do not leave feces in the yard or the park |
| Do not let the dog lick faces or wounds | Especially with infants and young children, immunosuppressed people, and anyone with an open wound |
| Regular deworming and external parasite control | It protects the people as much as the dog |
| Bite management | Wash immediately, seek medical care (for the person), report as required locally, and keep the dog's vaccination status verifiable |
| Disclose the risks of raw feeding | The feeding method should be reassessed when a pregnant woman, an infant or an immunosuppressed person is in the household |
| New dogs and shelter dogs | A quarantine period on arrival plus fecal testing plus a skin examination, before close contact with children in the household |

FAQ
- Is picking up dog feces really that important?
- Very important. Canine roundworm eggs are passed in the feces, mature in the soil, and infect people when swallowed, which can cause visceral larva migrans and ocular larva migrans, the latter capable of causing loss of vision in children. Hookworm larvae penetrate the skin directly and cause cutaneous larva migrans, a winding, red, swollen and intensely itchy lesion. Picking feces up promptly is the key point at which both parasites are blocked, in the yard as much as in the park; wash your hands after playing in sand or gardening as well.

- We have an infant or an immunosuppressed family member at home — is raw feeding a risk?
- Yes. Raw meat diets and the surfaces they touch can carry *Salmonella*, *Campylobacter* and pathogenic *E. coli*, which is a two-way risk for the dog and for the people living with it, and young children and immunosuppressed people may be more severely affected. If raw food is fed, hygiene requirements are at their highest: separate cutting boards and containers, wash hands after feeding, clean the bowl immediately; it is strongly discouraged when an immunosuppressed family member is in the household.

- I was bitten or scratched by my own dog — do I need to see a doctor?
- Every bite needs medical assessment, and injuries to the hand, immunosuppressed people and asplenic people warrant particular attention. A dog's mouth commonly carries bacteria such as Pasteurella multocida and Capnocytophaga canimorsus, wound infection can deteriorate quickly, and Capnocytophaga can cause fatal sepsis in immunosuppressed and asplenic people. The response is to wash immediately, seek medical care, and keep the dog's vaccination status verifiable; as a routine, do not let the dog lick wounds or faces.
