後牙大範圍蛀壞,不能只按材料耐磨度選擇樹脂、全瓷嵌體或牙冠。牙醫師需要先看蛀壞與舊填補移除後還剩幾面牙壁、每個咬頭的厚度與支撐、是否有裂紋、牙髓或根管狀態,以及您的咬合力會落在哪裡。二〇一七年的系統性回顧納入五項隨機試驗與九項觀察研究,發現剩餘齒質越少,修復失敗率越高;但整體證據僅低至中等品質,仍不足以用一條齒壁數量界線指定材料。 若周圍牙壁與咬頭仍有足夠支撐、邊緣可良好隔濕,直接樹脂可能保留較多齒質,也較容易局部修整。若某個咬頭已薄弱,單純把洞內填滿並沒有替它改變受力路徑;此時可討論覆蓋一個或多個咬頭的陶瓷 onlay/overlay。若缺損、裂紋或舊補綴已環繞多面,才會把完整牙冠納入評估。
