咬下或放開時出現短促疼痛，可能來自牙裂、鬆動修復體、咬合過度、牙髓發炎、牙周問題，甚至鄰牙或咀嚼肌的轉移痛。顯微鏡可放大牙面、修復體邊緣、髓室底與染色線索；配合局部咬合測試、透照、牙周探測與影像，能讓「疼痛在哪裡被誘發」變得更可核對。 但顯微鏡只能看見已暴露的表面，不能穿透牙本質，也不能單靠一條線判定裂縫深度。CBCT 對垂直牙根裂的統合敏感度約 0.70、特異度約 0.84，仍會漏掉裂縫或出現假陽性。 安全的診斷不是找一個萬能工具，而是讓多項互補證據朝同一方向收斂。
