E-Cigarettes, Cigarettes, and Heated Tobacco: The Full Evidence on Smoking Cessation, Harm Reduction, and Health Risks
The carcinogenicity and disease burden of cigarettes are firmly established. In supported smoking-cessation settings, nicotine-containing e-cigarettes can indeed help some adults stop smoking cigarettes, but they are not harmless and should not be promoted to adolescents, people who do not smoke, or pregnant women. Heated tobacco products, meanwhile, cannot yet be claimed to have been proven to reduce long-term health harms.

The conclusion in one sentence
The harms of cigarettes are clear. Nicotine-containing e-cigarettes can help some people stop smoking cigarettes in appropriate adult cessation settings, but e-cigarettes are not harmless. Distinguish “increased risk” from “certain to develop disease”
Long-term smoking greatly increases the risk of lung cancer, but this does not mean that every person who smokes will inevitably develop lung cancer. Nor can isolated cases of long-lived smokers prove, in reverse, that smoking is safe. Professor Lin used the principle that individual cases cannot overturn population-level probabilities to explain this point, which is consistent with the direction of population studies. Research on bronchial epithelium also shows that the frequency of somatic mutations rises with age and smoking pack-years before reaching a plateau. This offers clues for understanding individual variation, but “more effective DNA repair” and “better detoxification of tobacco-related harms” remain possible mechanisms rather than directly verified conclusions; Professor Lin’s original article likewise identified them as hypotheses. Assessing tobacco harm therefore requires more than asking whether someone will develop cancer. We must also ask how much the risk increases, how long exposure lasts, whether it continues, and whether the comparison group consists of people who have never smoked, cigarette users, or people who have quit completely. The carcinogenicity of cigarettes is established in medicine, and global burden-of-disease research continues to identify tobacco as a major source of preventable death and disease. E-cigarettes can help people stop smoking, but their appropriate use has clear boundaries
Among adults who smoke and receive cessation services, behavioral support, and continued follow-up, nicotine-containing e-cigarettes may be more effective for smoking cessation than nicotine replacement therapy. Subsequent living systematic reviews also support the conclusion that e-cigarettes can serve as a cessation tool for adults who genuinely want to stop smoking combustible cigarettes. This conclusion does not encourage everyone to start using e-cigarettes, nor does it mean that every product or pattern of use is effective. Professor Lin’s original article in fact acknowledged the cessation benefit seen under controlled conditions while emphasizing the importance of professional support, product management, and the intended users. If the headline-like statement that e-cigarettes “cannot help people quit smoking” is read as a universal claim with no exceptions, a reassessment based on today’s highest-level evidence points in a different direction from the literature. A formulation more consistent with current evidence is: “They can help some adults who smoke, but they are not an unconditionally safe option.” This reflects an updated literature and the evolution of science; it does not retroactively attribute a new claim based on later evidence to Professor Lin at the time he wrote. Observational studies conducted in real-world settings have found that some people who switched to e-cigarettes were more likely to return to smoking cigarettes later. Such studies, however, may be affected by the difficulty of quitting, motivation for use, and reverse causation. An association alone cannot establish that e-cigarettes inevitably cause relapse. The practical lesson is that if cessation is the goal, the priorities should be stopping cigarettes completely, continuing to receive support, and avoiding prolonged dual use. “Harm reduction” is not “no harm”; the comparator determines the answer
Compared with combustible cigarettes, e-cigarettes may reduce exposure to some harmful substances, and risk signals for certain disease outcomes may also be lower. Yet lower short-term exposure cannot establish that every long-term disease risk has fallen, much less that the products are nearly harmless. PMID: 42154468;PMID: 41702869
When the comparator is a person who uses no tobacco products at all, e-cigarettes still expose the user to nicotine, aerosols, and by-products of heating. When propylene glycol and vegetable glycerin are heated and inhaled, they can form harmful thermal-decomposition products such as formaldehyde, acetaldehyde, and acrolein. “Suitable for use in food” does not mean “safe when repeatedly heated and inhaled.” Testing of commercially available e-liquids has also found discrepancies between measured nicotine content and label claims, so product quality and regulation cannot be left out of the assessment. The error most important to avoid is turning “less harmful than cigarettes” into “worth trying for people who do not smoke.” For people who already smoke cigarettes and cannot quit by other means, switching completely is not the same as continuing dual use. Adolescents, people who do not smoke, and pregnant women are not the target population for a cessation tool
E-cigarette use among adolescents raises concerns about nicotine initiation, dependence, and subsequent cigarette smoking, while flavored products increase their appeal. Multiple reviews support a consistent association between rising e-cigarette use among adolescents, respiratory harm, and later initiation of cigarette smoking. “E-cigarettes can help adults who smoke to quit” and “e-cigarettes may lead young people who did not smoke to begin using nicotine” can both be true. Public health policy must preserve a regulated cessation option for adults while protecting adolescents, people who do not smoke, and pregnant women from exposures they otherwise would not have had. E-cigarette–associated lung injury: reasonable reservations at the time, followed by a crucial update
E-cigarette– or vaping-product-use-associated lung injury is a real and potentially very severe clinical syndrome. Patients may develop respiratory, gastrointestinal, and systemic symptoms; severe cases require hospitalization and respiratory support. At the time, many cases were associated with exposure to products containing THC. Professor Lin used these cases to warn that e-cigarettes could cause serious harm, and this core risk assessment is consistent with the case data. PMID: 31491072;PMID: 40427906
Early in the outbreak, Professor Lin’s skepticism toward a single cause, based on the data then available, was a cautious position appropriate to the state of the evidence at that time. Science subsequently advanced: data from bronchoalveolar-lavage fluid collected across multiple states greatly strengthened the association between vitamin E acetate and that wave of lung injury, particularly implicating illicit products containing THC. It is therefore no longer appropriate to characterize vitamin E acetate broadly as an unlikely suspect. Lower exposure from heated tobacco does not equal a lower long-term risk of disease
Studies of heated tobacco products have reported signals of lower exposure and changes in some biomarkers. An absolute denial that any exposure reduction occurs would therefore no longer reflect the subsequent data; this is another area in which the literature has been updated. Improvements in exposure markers, however, cannot be directly converted into confirmed reductions in cancer, cardiovascular disease, or mortality risk. PMID: 41281023;PMID: 40300839
The more defensible conclusion at present is that heated tobacco products have not been established as products that reduce long-term health harms, nor have they been adequately proven to be smoking-cessation tools. Practical decisions: the goal should be to leave combustible cigarettes behind, and ultimately all tobacco products
For people who do not smoke, the best choice is not to start using cigarettes, e-cigarettes, or heated tobacco products. People who currently smoke cigarettes and want to quit should first seek cessation services that provide follow-up and behavioral support. If, after an appropriate assessment, they use a nicotine-containing e-cigarette, the explicit goal should be to stop smoking cigarettes completely rather than to use both products indefinitely. The evidence as a whole does not endorse any tobacco product. Instead, it guards against two equally dangerous simplifications: describing e-cigarettes as harmless, and denying their benefit in specific adult cessation settings merely because they cause harm. An answer genuinely grounded in evidence must specify the comparator, the identity of the user, the regulation of the product, whether the switch is complete, and whether the evidence supports long-term clinical outcomes.
