根管器械分离是治疗中的技术事件，并不等同于牙齿立即失败。 真正要问的是：碎片位于哪一段、在弯曲前还是弯曲后、能否在显微视野下看到、分离前根管清洁到了什么程度，以及取出会牺牲多少牙本质。 在前瞻性临床研究中，取出与绕过合计的总体完成比例为 70.5%；碎片可直接看到时，该研究尝试取出，完成比例为 85.3%；碎片不可见时，该研究只进行绕过、未尝试取出，完成比例为 47.7%。 这些是特定团队与标准流程下的操作比例，并不是个别牙齿的保留率。 如果强行取出需要大量切除内侧牙本质，原本的金属碎片风险可能转变为穿孔、台阶或牙根脆弱的风险。
