根管治療後も透過像、腫れ、咬合時の不快感が残っていても、抜歯しかないという意味ではありません。 非外科的再根管治療では歯冠側から再度アクセスし、古い材料を除去し、見逃された空間を見つけて再び清掃することを目指します。 歯根端切除術では歯根の末端側から病変と根尖を処置します。 両者に上位・下位の区別があるのではなく、アクセス経路が異なります。 直接比較を行ったシステマティックレビューで見つかったのは5件の研究、追跡可能な歯 529 本にとどまりました。 大部分の研究はバイアスリスクが高く、メタアナリシスも実施できなかったため、著者らはいずれかの治療が明らかに優れているとは判断しませんでした。 垂直性歯根破折がある、う蝕が深く修復不可能である、または歯周組織の支持が不十分な場合にはじめて、保存する経路の合理性が失われる可能性があります。
